Notes: Our Spikes Ladies want other ladies to join their group of 19 members. Mews Centre is an accessible location and a walking track is provided for our use after the drop-in, at no cost. After city staff buzz you in, our room is the first hallway on your right, last door.

In the event that the widow or Beneficiary is not aware that a policy exists, a letter of inquiry should be sent to the above noted address, again providing the owner's Social Insurance Number and policy #83091.

Note: The Beneficiary of the insured person is stated in the person's application. The beneficiary designation may be changed, if permitted by law. The insured person must notify Sun Life Assurance in writing of the change. If no beneficiary has been appointed or if the Beneficiary has predeceased the insured person, payment will be made to the insured person's estate.
